Maximos 4 was a native of Dalmatia and was patriarch of Grado for twenty years; he was the successor of Primogenios 1 and was succeeded by Stephanos 155: Chron. Alt., p. 124 ("Maximus patriarcha, qui fuit nacione Dalmacie civitatis; qui vixit ann. XX"). Patriarch of Grado for twenty years, he was succeeded by Stephanus (Stephanos 155): John the Deacon, Cron. Ven., p. 87. Bishop of Aquileia; he attended all five sessions of the Lateran Council, from 5 October to 31 October 649, and in the list of bishops present is named in second place (after pope Martin, Martinos 6): Conc. Lat., p. 2, line 9 (Μαξίμου ἐπισκόπου Ἀκυλείας), p. 3, line 9 ("Maximo sanctissimo Aquileiense episcopo") (the first session), cf. pp. 31, 111, 177 and 247 (the Latin version only is repeated for the other sessions). At the first session he argued that the monothelete doctrine was contrary to the Council of Chalcedon and urged that the writings of Sergius, Kyros 2, Pyrrhos 1 and Paulos 2 be examined by the Council to establish that they were heretics: Conc. Lat., pp. 26-29, cf. p. 26, line 1 (Μάξιμος ὁ θεοφιλέστατος ἐπίσκοπος Ἀκυλείας) with p. 27, line 1 ("Maximus venerabilis episcopus Aquileiensis ecclesiae"). At the second session he ordered the reading of the libellus presented to the Council by Ioannes 471 and the other hegoumenoi and monks: Conc. Lat., p. 50, line 15 (Μάξιμος ὁ ἁγιώτατος ἐπίσκοπος Ἀκυλείας) with p. 51, line 15 ("Maximus sanctissimus episcopus Aquileiensis ecclesiae"). Later in this session he spoke accepting that the letter from the bishops of Proconsularis to the patriarch Paulos 2 confirmed the orthodoxy of their views and demonstrated that those of Paulos 2 were heretical: Conc. Lat., p. 95, line 16 ("Maximus sanctissimus episcopus Aquilegensis ecclesiae"), but. cf. p. 94, line 16 (the Greek text attributes this statement to pope Martin, Martinos 6). At the fourth session, following the reading out of statements of faith from the five ecumenical councils, he spoke at length condemning the views of Kyros 2, Sergius, Pyrrhos 1 and Paulos 2 as inconsistent with those accepted at the councils: Conc. Lat., pp. 234-245, cf. p. 234, line 7 (Μάξιμος ὁ ἁγιώτατος <ἐπίσκοποσ> Ἀκυλείας) with p. 235, line 7 ("Maximus sanctissimus episcopus Aquileiensis"). At the fifth session, after the reading of passages from the Fathers and from heretics and and after Martinos 6 had spoken, Maximos 4 also spoke condemning the views of heretics: Conc. Lat., pp. 344-353, cf. p. 344, line 1 (Μάξιμος ὁ ἁγιώτατος ἐπίσκοπος Ἀκύλειας) with p. 345, line 1 ("Maximus sanctissimus episcopus Aquilegensis ecclesiae"). He subscribed the definition of the faith agreed by the Council at its fifth session: Conc. Lat., p. 390, line 15 (Μάξιμος ἐπίσκοπος Ἀκυλείας), p. 391, line 15 ("Maximus episcopus sanctae Aquileiensis ecclesiae").
