Kosmas 12 was proclaimed emperor in 727 when the people of Hellas and the Islands rebelled against Leo III (Leo 3) because of his attitude towards icons (Κοσμᾶν τοὔνομα ἐφ' ἑαυτοῖς βασιλεύουσι: Nic. Brev. de Boor 58, Mango 60.11; Κοσμᾶν τινα συνεπόμενον ἔχοντες εἰς τὸ στεφθῆναι: Theoph. AM 6218); the rebels were defeated at sea near Constantinople (on 18 April 727; Theoph. AM 6218) and Kosmas 12 was captured and beheaded (καὶ ἀποτέμνονται τὰς κεφαλὰς Κοσμᾶς τε καὶ Στέφανος: Theoph. AM 6218); among his supporters who also perished were Agallianos 1 and Stephanos 1: Nic. Brev. de Boor 58, Mango 60, Theoph. AM 6218.
