Euthymios 12 was the son of an unnamed patrikios and magistros (Anonymus 561); he was the hegoumenos of an unnamed monastery; accompanied by the monk Thomas 58, he visited Ioannikios 2 for his blessing; on this occasion Ioannikios 2 foretold the death of Thomas 58: Sabas, Vita Ioannicii 35 (μετὰ Εὐθυμίου ἡγουμένου, υἱοῦ γενομένου πατρικίου τε καὶ μαγίστρου), Petrus, Vita Ioannicii 65 (ἅμα Εὐθυμίῳ, τῷ υἱῷ τοῦ μαγίστρου). At the time Ioannikios 2 was living near to the monastery of Antidios, and the date was probably late in the reign of Michael II (Michael 10) or soon afterwards; see Ioannikios 2. Cf. Euthymios 3.
