A priest at Edessa, Theophylaktos 5 was elected patriarch of Antioch in 744 after the death of Stephanos 9; the caliph Marwan II (Marwan 2) not only gave permission but insisted that the Arabs treat him with proper respect: Theoph. AM 6236 (χειροτονηθῆναι Θεοφύλακτον, πρεσβύτερον Ἐδεσηνόν, πατριάρχην Ἀντιοχείας). He died on June 29 (Daisios 29), 751: Theoph. AM 6242 (καὶ τῷ αὐτῷ ἔτει ἐκοιμήθη ὁ ἁγιώτατος πατριάρχης Ἀντιοχείας Θεοφύλακτος μηνὶ Δαισίῳ κθ'). His successor was Theodoros 10: Theoph. AM 6243.
