The name and titles read: Κωνσταντινάκῳ βασιλικῷ διακόνῳ καὶ σκευοφύλακι. The name was read by Laurent as: Κωνσταντινακίῳ, but Zacos and Veglery confirm the reading as either Κωνσταντινάκῳ or Κωνσταντινάκῃ.
