Ignatios 12 was a former disciple (μαθητὴς) of the patriarch Ignatios 1; he deserted Ignatios 1 and supported the patriarch Photios 1; he held the post of archon of the monasteries of Propontis, then was made metropolitan bishop of Hierapolis by Photios 1 (καὶ ἄρχων μὲν τέως τῶν μοναστηρίων τῶν κατὰ τὴν Προποντίδα, ἔπειτα καὶ μητροπολίτης ὑπ' αὐτοῦ τῆς Ἱεραπόλεως καθίσταται); it was he who in c. 863 told Photios 1 about the altar on the island of Plate which had been re-erected by Ignatios 1 after being overthrown by Russian raiders: Nicetas, Vita Ignatii 532B. Since he became a metropolitan bishop, his see was Hierapolis in Phrygia Pacatiana, not that in Phrygia Salutaris.
