Felix 5 was archbishop of Ravenna; he was ordained by pope Constantine (Konstantinos 136) (708/715), but the documents (cautiones) which he deposited with the pope were irregular and unsatisfactory; when Ravenna rebelled and was captured by Theodoros 168, Felix was detained in fetters on board ship and sent to Constantinople, where he was blinded and exiled to Pontus ("isdem archiepiscopus, lumine privatus, dignam factis recipiens poenam, exul in Pontica transmissus est regione"): Lib. Pont. 90. 2. The date was in or before 711, when Justinian (Ioustinianos 1) was still emperor. After the overthrow of Justinian and the accession of Philippicus (Philippikos 1), Felix 5 returned from exile repentant and in spite of his blindness returned to his see; he provided the normal documents and declaration of faith for the papal archives and so received pardon and was reconciled ("et solita quae ab universis in scrinio episcoporum fient indicula et fidei expositiones et hic confessus est, sicque reconciliationis promeruit absolutionem"): Lib. Pont. 90. 9, cf. Agnellus, Lib. Pont. Eccl. Rav. 150. On the documents required of him, see Duchesne, Lib. Pont., p. 393, n. 2, and H. Foerster, Liber Diurnus romanorum pontificum (Bern, 1958) III 6, 7, 8.
