Andreas 16 was bishop of Eueragapa in Phrygia Pacatiana; in 692 he attended the Quinisext Council in Constantinople and signed the canons: Ohme, Bischofsliste, p. 163, no. 170 (= Mansi XI 1001) (Ἀνδρέας ἐπίσκοπος Εὐεραγάπων Πακατιανῆς).
